I hereby certify that I am the rightful owner/keeper/caretaker/custodian of the cat(s) who is/are the subject of this Cat Surrender Form, hereinafter referred to as "cat". I hereby surrender any and all property rights to the cat. I certify that no other person has a right of property to the cat and I have pursued all legal remedies to find an owner of the cat. I understand that by surrendering my property rights to the cat, the cat may be transferred into the custody of an animal shelter.

I also hereby certify that the cat has not bitten or scratched a human or other animal within the past ten (10) days. I understand that once I relinquish the cat it may not be available to be returned, except as previously arranged with CCI. I further certify that I have read and understand the terms of this Cat Surrender Form.
