I am the adopter of the cats listed below. I am returning it/them to Candy's Cats, Inc. (CCI) for the reasons stated within the agreed upon date. Upon completing the physical transfer of the cat(s) and signing this request, ownership of the cat(s) reverts back to CCI and the Adoption Contract previously entered into becomes null and void.

Pursuant to the Adoption Contract entered into by me with CCI, the refund clause states: I have 48 hours to change my mind for a full refund. Within 48 hours, I will: Call CCI in order to return the cat in a carrier with all original paperwork and any other items provided to me by CCI or their representative to whatever agreed upon location. After 48 hours, there will be no refunds but CCI gets the right of first refusal if I cannot keep the cat for any reason after the 48 hour period. Refunds are approved by the Board of Directors. The cat, medicine, medical records and other items provided to me for the cat must be returned in good condition and the refund will be mailed or credited within 30 days of the return. If the cat is deceased within the first thirty (30) days, veterinarian proof of the cause of death must be provided. The cat(s) has/have been seen by my vet and are current on their vaccines. CCI requests that the cats be combo tested prior to returning to our foster homes, when possible. If the cat(s) is/are not current on vaccines, CCI requests they be updated prior to returning them. Depending on the age of the cat(s), at a minimum, a combo test, fecal and rabies vaccine are needed. (This helps us to keep our other costs down.) By signing this agreement, I am providing vet records and authorize CCI access to the vet to obtain the vet records. I will provide food that the cat(s) is/are used to eating and any cat supplies to help with the transition. The cat(s) name(s) we provided and the reason for the return is/are as follows:*

I hereby release, waive, discharge, and covenant not to sue CCI, its employees, agents, members, directors, officers, or any affiliated entities thereof, from all liability to the undersigned, his/her personal representative, assigns, heirs, next of kin, guests, and/or invitees for any and all loss or damage, and any claim or demands therefore on account of injury to the person or property or resulting in death of the undersigned or anyone else arising out of or related to the handling of an animal from CCI. By signing this agreement, I agree to release, indemnify, and hold harmless CCI, as well as all of its administrators, employees, agents, representatives, officers, directors, successors, other volunteers, vendors, store locations, off-site venues and locations, etc. from all losses, claims, theft, demands, liabilities, grievances, causes of action, and/or expenses, including but not by way of limitation, all liability for property damages and personal injury of every kind, nature or description arising or which may hereafter arise, known or unknown, out of my temporary participation with CCI to return the adopted cats, or out of my presence on any site, event or home used to further the cause of CCI.
